Intake Information & Referral Services
All requests for service and eligibility determination are initiated through our intake information and referral service.
Our coordinators receive referrals from individuals, families, and various community resources. If the individual is determined to be eligible for services, and the need for ongoing services is indicated, we will assist the individual through the intake process. If the individual is determined to be ineligible for services, they will be assisted in seeking assistance from other agencies/organizations in our community.
Eligibility by Age
Eligibility for children from birth to age three
All referrals for children birth to age 3 are made through Help me Grow in conjunction with Williams County Board of DD. The Williams County Board of DD works in partnership with Help Me Grow, to provide developmental evaluations to children who have a suspected delay or who have a medical condition, which has a high probability of resulting in a developmental delay. Once Williams County Board of DD evaluation team completes the assessment and determines your child is eligible for services, you are given options for supports. One of the many choices for parents is Early Intervention service through Williams County Board of DD. Children without a medical diagnosis will have their eligibility re-determined annually. Children with a medical diagnosis will be re-determined by their third birthday.
Eligibility For children ages three to five years old
Children age 3 to 5 are eligible for support if they have a significant delay in one area of development, or less significant delays in two developmental areas reported in the Multi-Factored Evaluation (MFE).
Eligibility For children ages six to fifteen years old
Children age 6 to 15 are eligible for Williams County Board of DD services if they have a documented history of life- long developmental disability and significant functional limitations in at least three areas of the Children’s Ohio Eligibility Determination Instrument (COEDI). Areas evaluated; self-care, mobility, receptive and expressive language, self-direction, capacity for independent living, and learning. Eligibility is re-determined at age 16 for continued services.
Eligibility For children ages sixteen and up
Individuals age 16 and above are eligible for Williams County Board of DD services if they have a documented and diagnosed history of life-long developmental disability occurring before age 22 and significant functional limitations in at least three of seven life areas on the Ohio Eligibility Determination Instrument (OEDI). Areas evaluated; self- care, mobility, receptive and expressive language, self-direction, capacity for independent living, learning, and economic self-sufficiency. Eligibility status may be reviewed at any time if a significant change in diagnoses, impairment or skill has occurred.
