How we can help

Service Coordination is a process of service planning to help families whose youth receive services from multiple systems. It brings the youth, their family and service providers together to foster communication, coordination and collaboration.  The process is family-focused and strengths-based.  Therefore, it results in a unique set of community services and natural supports individualized for the youth and family to achieve a positive set of outcomes.

This document explains the purpose, eligibility and processes involved in our local service coordination.

Early Intervention

Early Intervention is an Ohio-wide system providing coordinated services to parents of infants and toddlers with disabilities or developmental delays. Services align with the philosophy that young children learn best from familiar people in familiar settings. Families work with their local team to develop a coordinated plan to use their existing supports and resources – and build upon them – to learn to enhance their child’s learning and development.
