Position Description:
This SSA position will work directly with children and families from birth thru age 5, providing service coordination and transition services to families from the Early Intervention direct service model to the preschool system with SSA case management. Additional duties include determining eligibility based on developmental screenings or preschool Part B eligibility. Responsibilities include billing both Service Coordination, as part of Part C, and Targeted Case Management. This position will work directly with both the Early Intervention and SSA teams, and work to educate the community on services provided by WCBDD. This is a part-time hourly position working 30 hours per week, 12 months per year.
Qualifications:
The Early Learning SSA shall possess a Bachelor Degree in social work, special education, or related field; must possess or be able to obtain and maintain a Ohio Department of DODD Service and Support Administration Certification and Early Intervention Service Coordinator Certification; experience working with individuals with disabilities is preferred; familiarity with Medicaid related programs and knowledge of community resources preferred; must possess a valid driver’s license, reliable vehicle and be insurable; must pass a criminal background check; must be able to work a flexible schedule. This position also requires excellent judgment, direct communication, tact, problem-solving, sense of fun and adventure, flexibility, multi-tasking, and ability to lead group activities.
